Alternatively you can call the Grigioni Foundation (+39 02-66710423) or you can talk about it with your neurologist during one of your follow-up examinations at the ICP Parkinson Institute.

You will be given an Informed Consent Form, subdivided into three parts:

1)      1) a membership form that must be filled in with the personal data of the donor;

2)      An information sheet about the purpose of the project, which you should read

3)      Information about personal data processing

After having read the form you can clarify any doubt on BTN by talking to the BTN coordinator. Then you can join the BTN project by signing the Informed Consent Form.

Should you not be a patient attending the Parkinson Institute, you will be asked to provide personal data and your medical history regarding PD or your health in general, as appropriate.  You will be asked whether you have 1^st degree relatives (parents, siblings, children) with PD.

After joining and registration as a donor, you will receive a plastic card and a scroll, which certify that you have joined the project.
